Home Care Services

 

Based on need and the availability of trained providers, the home care services that may be provided are:

 

Light house cleaning                        Essential shopping                     Teaching instructions

                             Cooking                                              Laundry                        Non-medical personal care


Physical Fitness, Health Promotion and Information

 

The program provides speakers, videos, exercise equipment, classes, health screening, nutritional information, disease prevention and our walking track at Judy Park.


 

Outreach, Information and Assistance

 

  • Search out senior adults and assist them in gaining access to the services and benefits they require.

 

  • Evaluate the situation of senior adults and establish service goals to meet their needs.

 

  • Provide one-on-one counseling when assessing needs.

 

  • Coordinate between SCCC and other agencies to assure access to the appropriate service provider.

 

  • Follow-up to assure the prescribed client programs are meeting the client’s needs.

 

  • Provide the staff training required to meet quality assurance standards.

 

Transportation

 

Essential transportation for senior adults and the physically disadvantaged to give them access to services for their health and well-being.

 

Transportation is coordinated with Department of Social Services (DSS) Medicaid clients.
